Sun Valley Hospice provides comprehensive, compassionate care for persons with a life-limiting condition or illness. Our interdisciplinary team uses a holistic approach to promote the patient's comfort and dignity. Recognizing the family as the unit of care, we support the patient, family and caregivers by addressing physical, psychosocial, and spiritual needs. Care is provided wherever the patient lives; including the patient's own home, residential care homes, skilled nursing facilities, assisted living or independent living facility. |

Comprehensive Services |

Hospice care is provided on an intermittent basis 24 hours per day, seven days a week based upon an approved Plan of Care involving the patient/family/caregivers, the attending physician and the hospice team. Prescription medications, supplies and medical equipment related to the patient;s terminal illness are provided at no cost to the patient or family. |

Registered Nurses & Licensed Practical Nurses. Certified Nursing Assistants. Licensed Social Workers. Pastoral Counselors. Speech, Occupational and Physical Therapists. Dieticians. Bereavement. Volunteers. THE COST OF HOSPICE CARE |

Hospice Care is covered by Medicare. Most major insurance companies also have a hospice benefit. Please call Sun Valley Hospice for more information or to request a hospice evaluation. |
